So what happens when something terrible happens to you, but no one knows your information?

I'll let you know. 

You'll be headed to the closest hospital, but it will likely NOT be the one you trust. 

You alone will be responsible for giving your entire medical history. This can be pretty hard to do in the middle of a heart attack or a stroke. 

So, If you're unable to communicate or remember everything, you will be at a high risk of complications. 

Why? Because the doctors won't know about your drug allergy or the medication you just started taking. That medication may be the cause of what’s happening to you, or it may interact with the Doctor’s treatment.

So, how can you fix this? 

Make a list! 

I’ve created a simple template for you that gives paramedics and Doctors all the information they need. This notecard is not just a convenience; it's a necessity in an emergency. 

It’s as easy as print, complete, fold and post.
